claim form (in civil proceedings) A formal written statement setting out details of the claimant, defendant, and the remedy being sought. The claim form may also contain details of the claim (the particulars of claim); alternatively, these can be served separately. Since the introduction of the Civil Procedure Rules in 1999, the usual method of initiating civil proceedings is by issuing a claim form under Part 7 of the Rules; all previous methods (e.g. writ of summons, originating summons) have now been rendered obsolete.
